But a Mandibular Advancement Device functions differently. You wear it in your mouth during sleep, and it is designed to hold the lower jaw forward as you rest.

Most MADs are custom fitted, using what is called boil and bite technology.
This is a process that you can usually do right at home, and it must be done before you use the device.
Basically, you submerge it in either very hot or boiling water. You then remove it from the water, and bite down on it while it is still warm to form an impression.
This impression helps you to customize it so that the fit is exact and precise.
There are a few MADs that aren’t fitted this way. But for the most part, this is how most popular MAD devices are custom-fitted.
After fitting, MADs must be worn every night to be effective. They work by holding the lower jaw forward during sleep, which helps to create more room in the upper airway.

TSDs and TRDs – Tongue Stabilization Devices
TSD stands for Tongue Stabilization Device. They can also be called TRDs, however… which stands for Tongue Retaining Device.
This is another type of anti-snoring mouthpiece, though it doesn’t really resemble a sports mouthguard like an MAD.
This type of device also doesn’t require any sort of fitting before use.
The way it works is pretty simple. It generally consists of a cross guard that secures it against your teeth, and a spot where your tongue can be secured… usually via suction.
You place the device in your mouth, stick your tongue into the suction mechanism, and let the device keep your tongue held forward during sleep.
This does something very similar to what an MAD does by helping to keep the upper airway clear and open during sleep, as it prevents the tongue from falling backwards and ‘collapsing in’ on the airway.
But despite its similarities, it does function a bit differently… and it doesn’t hold the jaw forward like an MAD does.
Upsides And Downsides
As with the MAD, there are upsides and downsides to the tongue stabilization device.
The biggest upside is that it tends to be almost as effective as a mandibular advancement device, but with far less bulk.
The downside is that it doesn’t advance the jaw… so the effect isn’t quite as strong. Very severe cases of snoring may not be able to be solved with a TSD… even though an MAD may still be able to help in such cases.

Occasionally, you will end up with both problems… snoring in the nasal passages and in the upper airway.
